What are the most effective treatments for male pattern hair loss?

There are several evidence-based treatments available for male pattern hair loss. It is important to note that all of the below treatments require around 6-12 months to see the full benefit, although benefit can often be seen from 3 months. Stopping treatment may lead to loss of the benefit that has been gained. These treatments include:

1. Minoxidil

Minoxidil is a widely used topical treatment, and can be taken also as a tablet. Minoxidil stimulates hair growth by increasing blood flow to the scalp and nourishing hair follicles. The topical preparation is available in liquid or foam form, typically at a 5% concentration, however can be compounded at higher concentrations, and is applied directly to the affected areas of the scalp, typically twice daily.

  • Effectiveness: Minoxidil can slow down hair loss and promote regrowth, particularly on the crown and temples.
  • Considerations: Patients may experience an initial shed of hair before benefit is seen.

2. Finasteride

Finasteride is a prescription tablet which can also be administered as a topical preparation to the scalp. Finasteride addresses the hormonal cause of male pattern hair loss by reducing DHT levels in the scalp. It works by inhibiting the enzyme 5-alpha-reductase, which converts testosterone into DHT.

  • Effectiveness: Finasteride helps prevent further hair loss and can even stimulate new growth.

  • Effectiveness: Studies suggest that LLLT can moderately improve hair density and thickness when used consistently, especially when combined with other treatments like minoxidil or finasteride.
  • Considerations: LLLT is generally safe but requires regular use over time for noticeable results. I personally find the other treatments more effective, however this is a simple and safe adjunct.

5. Hair transplant surgery

Hair transplant surgery offers a permanent solution for male pattern baldness by relocating healthy hair follicles from areas with dense growth (usually the back or sides of the scalp) to thinning or bald areas.

  • Techniques: The two main methods are follicular unit transplantation (FUT) and follicular unit extraction (FUE). Both provide natural-looking results when performed by an experienced surgeon.
  • Effectiveness: Success rates are high, with visible improvements typically seen within 6 to 12 months after surgery. However, medical therapies are important alongside transplantation, as the male pattern hair loss process will likely lead to continued thinning over time if not adequately treated. Additional hair transplant treatments may be needed to maintain overall density.

Can combining treatments improve results?

Yes, combining treatments can often yield better outcomes than using a single therapy alone. This approach targets different aspects of hair loss for a more comprehensive effect:
